Professional Production Registration

Due to COVID we are requiring that production staff keep a DAILY log on ALL cast, crew and ALL other set visits or contacts for EACH day. This may assist in contact tracing if needed. This includes daily delivery persons for gear, craft services, expendables, etc.

Please complete and submit the registration below. All fields marked * are required. All specific data collected will be deemed private and will not be shared without consent of the production. The information is used for reporting purposes only.

You may be contacted by Film Columbus regarding your registration. From there, you will be directed to the appropriate municipality permitting office(s) if needed.

This registration is for Professional Productions only. Students do not need to register with the Film Columbus office.
